Therefore, we may participate with your insurance plan (even if it is not listed above), as a “Third Party” contracted provider through partnership arrangements. Please feel free to provide our staff with your specific insurance information, to further verify if we are considered “In Network” with your plan. Furthermore, In the event we do not participate with your insurance company, we expect all fees to be paid in full at the time service is provided. As a courtesy, our office staff will submit a claim to your insurance company, and they will send any reimbursement directly to you, if you have “Out-of-Network” coverage under your dental policy.
